Posterior belly—mastoid notch of temporal bone

Anterior belly—inner side of inferior border of mandible near symphysis

Insertion

Intermediate tendon attached to hyoid bone

Nerve

Posterior belly—facial nerve

Anterior belly—mandibular division of trigeminal

Actions

Raises hyoid bone, depresses mandible, moves hyoid forward or backward
